Emphysema is low compliance or high compliance?

I know it is caused by rupture alveoli, so it has poor recoil which means low compliance??

The TEAS will likely not have a scored question on directly applying lung compliance. If sacs are physically damaged and destroyed, they will not be able respond appropriately. They might have more surface area because they are damaged, but they won’t be able to stretch.

So yes, you are correct! Ruptured alveoli = poor recoil = low compliance.
